We work with local authorities, NHS Integrated Care Boards, community health teams, and self-funders across England. Making a referral is straightforward — contact our team to discuss the individual's needs and we'll guide you through the process.

Step 2: Assessment

We carry out a comprehensive needs assessment — visiting the individual if possible — to ensure we can provide the right level of care and develop an accurate, personalised support plan.

Step 3: Placement & Transition

We plan a smooth transition, involving the individual and their support network every step of the way. Our teams ensure the move-in is positive and well-prepared.
